If it's not condescension (which I imagine it usually isn't), it's fondness. It's strange how society so sharp delineates different forms of touch, that a gentle stroke on the shoulder should be so positive, but an insult if moved ten inches up and over, to the top of the head (okay, it's not ten inches for MOST people, but it's not my fault you all have such puny heads). There may also be an element of almost parental fondness to it, though, even if it's not a condescending thing.

Well, it CAN be condescending sometimes - though it's usually obvious when it is - but it can also be a way of showing care and comfort while specifically avoiding any presumption of sexuality - i. e., he wants you to know he cares, but doesn't want you to think he's just trying to get into your pants by doing so. This doesn't mean he doesn't WANT to get into your pants - it just means that he's not going to do anything about than until he knows YOU want it to happen too.
