Hard to tell, but I'm guessing it's probably played up for emphasis most of the time.

Some gay men actually act like that because that is their personality and others fake it but I can't speak for the group of gay men. (Also I would like to point out that them being "girly" is the wrong word to use maybe just say feminine.
