Well that wasn't the exact picture, but the same outcome.

If it was on purpose I'd be upset. I wouldn't feel trapped if my s/o got pregnant. I don't have sex with anyone I'm not willing to have a family with. I would be worried about her not being responsible enough to remember to take her birth control though. If she forgets stuff that important I would be worried.

This was actually gonna be my comment. And @spartan55 can see I agree with the comment he wrote under "what guys said" but I also agree with this one. I wouldn't leave nothing up to chance. I would pull out too while wearing the condom... Sure would. Sure would. I'm surly not about to complain over spoiled milk either. That's not going to move the needle. She made a mistake but she's my girl and I am with her every step of the way. We would grow from it and become better individuals. There are multiple lessons in what happened here.

I don't think one is wrong. There is a ton of wisdom in both. I can't say anyone is wrong because we all have different visions for what we want our relationships to look like down to the details.

@spartan55 That is smart there is a also birth control women can use that is longer acting like injections if they are prone to forget, but as @dayspring said if it is that important not to have an unplanned pregnancy than it's best to take all precautions possible but I think also think some women are going to forget, not do it intentionally nand even forgetting once might be all it takes to need a back-up method for a few days.
