So a guy I've known since childhood (gave me a real bracelet when we were 11) and I recently hooked up/hung out. He's only had 1 other relationship 10 years ago. When I told him that his friend keeps telling everyone I'm his girlfriend and asked him if it bothers him he said "no" and he admitted to me he has always liked me, he also kissed me before he left so does this make us an item? We act like boyfriend/girlfriend even before we hooked up lol. I am not pushing for anything, but he is also a very shy guy.

So I got brave and flat out asked him if it's ok I call him my boyfriend and he said yes it's ok so I guess that's a pretty clear answer.

If that's the case, then just tell him. If you were going to scare him off, it would have been long before you had that level of intimacy with him.

Be direct. Guys are simple.

You could start by asking him if he would want a relationship with you. If he's unclear, because he's shy, then state what you want.

If he's unclear, because he's being shy, then that's a yes.

Most guys prefer a direct approach. We don't like hints, or games, or test.

So just come out and say it.

Also, when I cuddled up next to him and asked him if its OK "he smiled, blushed and nodded" that's just how shy he is, but he also hasn't been with anyone since his first girlfriend 10 years ago. I asked him if he just wants sex or genuinely likes hanging out he said "he likes hanging out" and he literally sat staring into my eyes smiling for a good minute or 2 and during "activities" he just stared right at my face the whole time, gave me a kiss on the lips before he left (he stayed for 9 hours, even watched a movie after "activities") and said "no" it doesn't bother him that everyone thinks I'm his girlfriend. We even drove around town in his car for a bit.
