There are lots of reasons a guy might stare at a woman.

Maybe she has huge boobs.

Maybe she's really hot.

Maybe she's really tall,

Maybe she looks familiar, but the guy can't remember where he's seen her before.

Maybe she looks exactly like someone the narrator accidentally hit with his car late one night, and then didn't call the cops because he'd been drinking, and he drove away and left her for dead, and now he's trying to figure out if this is the woman he hit, who somehow survived, or if this is her twin sister out for revenge.

Or maybe she's wearing a nice shirt, and the guy is wondering where she got it.

It could be lots of things.

Yes, it can happen if the breasts are actually protruding part of the body. Will people stare at me if my hair looks unusual, yes.
If there is a very beautiful woman in overall appearance, no matter how her breasts are, the eyes are drawn towards her too once if she comes in front of the eyes.

It's certainly possible. One doesn't have to be attractive to get stares. Funny looking people get stared at, too
