That's up to the patient, doctors, and (if applicable) the patients legal guardians to decide.
I had spine surgery as a child, and while the risk of paralysis was very low, it was still there. It was up to me and my parents (with advice from doctors) to decide if the surgery was worth the risk. As it should be.

Depends on the surgery. Usually the chances of something like that are extremely small
