We are only looking at what it actually costs the doctor not what the charge the patient or bill to the government

It depends on how much the doctor spent on new fancy equipment during those years. Whenever they buy new stuff, they raise their prices.

It also depends on how many people in their office they have to pay, and how much their lease costs.

If you are talking about $100 for what it actually costs the doctor to do the exam, that is VERY expensive. It shouldn't cost anywhere near that much.
