Crazy hard work dehydration for the photo. While it doesn't look bad its fake as in thats not a permanent look even for this specific guy. So bassically it just sets unrealistic goals.
Henry cavil explained it well in an interview.

  How a scene/photo with tones muscles like that are not CGI, but its still fake because looking like that is not a standart and healthy to achieve for longer time. Even when you are in top shape.

Most likely photoshopped. Because your body fat has to be pretty low to get that defined.
