Which one of these weight-loss pitfalls do you think happens the most often?

I would say underestimating calories consumed happens more, but I've seen numerous pitfalls that happen even moreso.
The biggest of which I've seen happens specifically in the case of women who follow diets that end up in too drastic a reduction of calories. Eating below BMR can lead to severe health problems as well as stunt results.
Another common one I've seen is people who try to stack "incompatible" workout plans. For example adding a high intensity weight training regimen while doing Insanity. Muscles need rest to strengthen and grow and not giving them ample rest can stunt results and lead to increased risk of injury.
Both of these I feel are more common--and are bigger overall problems--than the ones you mentioned.
Unless you count calories and do research I think both are of course over estimated.
Counting calories is more common than researching calories burned I believe.
But if people didn't eat so much they wouldn't gain so much in the first place.
I think they both may occur as often, but I chose A because of a girl friend of mine. She works out constantly and is very athletic, but is still a classic apple shape with a stomach and thicker arms. She doesn't know why she can't lose that last bit and has decided it will just never come off. However, I know it's because she's eating a lot more calories than she thinks she is. The foods she thinks are "healthy" actually contain a lot of bread and sugar. She eats way more carbs than she should, with most of her meals consisting of bread, potatoes, etc. and is a chronic baker who eats sweets too often as well. However, since she doesn't eat very much, and thinks she's just eating mashed potatoes instead of McDonald's, she's eating healthy.

But that's why I always over estimate on them. It's better to be over than under I think.

Because so many "healthy" fad foods. are so high in calories. But people ignore it and are all "oh it's organic it can't be fattening"
