Depends when, why, where, how it's said and who it's said to. If it's a military recruit saying it to a drill sergeant it's not only rude but it also shows a lack of discipline. However if it's two friends who are okay with it then there is nothing wrong with it.

Usually, but not always. Kinda depends on context and tone.

The point of the word is usually to condescending mock someone for missing something that the speaker perceives as being blantantly obvious.
