Heavy Scottish can be difficult, as well as some Australian dialects (although that’s more some of the slang they use than anything).
Australian English rural vernacular. Sometime this comes up in documentaries. To me it sounds worse than London's Cockney.
American - pretty much any American accent is pidgin English.
Might as well call it a foreign language.

Some people I've met that immigrated from India, Pakistan, and China have a bit of a hard accent to understand at times, at least from my experience.
