do you want her to touch your hand gently or be playful and hit you back? if she hits you back does it turn you off? I've done both I've fist bumped him gently and then I've punched him hard just to be flirty, but he seemed annoyed by it...what gives?

No. I have shown affection in other ways just to touch someone but never specifically these. Also if I hit her and she hits me back that's awesome and we'll wrestle and then ... well anyway it's awesome.

As for your manfriend it totally depends on his mood. Sometimes people really don't want to get punched for no reason as a means of affection. Just learn to read the timing and you'll win every time. Sometimes it's hug / lay head on you time, sometimes it's fisticuffs!

Communication is key! Be cool and apologize. Or if you did apologize just explain yourself. Or if you did that cuddle him and rub the spot and say "YOUR AMAZING TO TAKE MY PUNCH AND NOT DIE". This will incite laughter, which is great for healing all kinds of wounds, and building resistance against new ones.

No, but I'd bet some people do. A girl tried to fist bump me yesterday, and I figured that's what she was up to. I'm a little more obvious with my touching. I'll playfully massage their shoulders when they look like they are having a hard time, or put my arm around them and draw them in closer when explaining something to them, or grab their hands/wrists when we are talking face to face, etc. To me shaking hands means nothing as far as flirting is concerned.

It's not a way I flirt, but it is a way girls flirt with me. They tend get in my space and try to rub against me, lean on me, give me little playful punches and pushes to the shoulder, stick out their hand for a high five or a fist bump, or throw things at me to get me to react to it, etc.

No, I tend to be far too obvious for something like that. If I want to touch you, you're going to know I want to touch you. I don't go in for making excuses to make slight contact, doesn't make much sense to me.
