I have a few questions

1. When is too soon
2. What is the best approach, should she say Where do you see us dating going? What are your plans with us?

I ask this because many guys have no iintentions in dating, some do it for fun, and some don't plan on leading to marriage, However I don't want to be with a commitment phobe, I do understand that it could come off needy if I don't phrase it right, but if he's thinking on the same page as you should he really be afraid? Or annoyed? What do you think?

my advice is not to ask him directly but to tell him your point of view and then getting his opinion on it.
For example you could phrase it like this:

Listen, we have been dating for X amount of time and I think you are a really cool person and getting to know you has been loads of fun. It is too soon for me to know for sure if this is relationship material, but I'd like to see how it progresses and if we are really compatible. However I don't like to waste my time with someone who is not on the same page as I. So before I fall in love with you, please let me know if you are open to the idea of keeping this going and if it goes well, if you can see yourself in a relationship in the not so distant future with me, say in X amount of months. If you are just in it for fun then I will have to think about it if I should continue to hang out with you.

Depends on your relationship... how deep it is... how serious it is.. etc

If you are not comfortable with him to ask it... maybe it is too soon?
