Removed is off by a generation. My mom's cousins are cousins once removed for me. Second cousins are children of your parents cousins.
Twice removed are my mom's cousins relationship to my cousin's daughter. Their children's children are third cousins.
It's determined by shared ancestor. Cousins share grandparents, second cousins share great- grandparents. Once, twice, etc. removed share them, but at different levels. My grandmother is their great grandmother or vice versa.
