I suppose paranoia is anxiety driven. You feel personally threatened. In contrast intuition is making a judgement call about good, bad, or neutral outcomes without really impacting your emotional state. You can understand the truth of what is going on without solid evidence immediately, but you are detached from your judgement.

By looking at your track record. If you're usually right, it's intuition. If you're often wrong, it's paranoia.
