I don't think it's ever a surprise when they ask. Before hand they're usually acting a bit suspicious, your friends are trying to get your ring size. I think you will have a suspension of what is about to happen.
Before it happends you will have a few days or weeks to think about it and you probably have thought about it before if you have been together for a while. "Do I see myself marrying this person?".
So I think I would be able to say yes in the moment itself.

Because we had already lived together for five years, I certainly didn't need any time at all in accepting my then boyfriend's proposal of marriage... We actually celebrated our fifth wedding anniversary last November.

It’s extremely simple, if you need time to think about it, you’re not ready to get married. Even people that do say yes on the spot don’t even realize they aren’t ready, which means if you have to think about it you really really REALLY aren’t ready.
