For first dates, I always preferred rowdy places like pubs. I think it helps to let loose and have some fun. Then later if we get more comfortable and serious about each other, switch to quiet joints like candlelit dinners with wine. Yet initially I like a rowdy and fun environment.
Oh sorry, I got all caught up in environment that I forgot about the food. Yet typically pub food like fish and chips, pizza, fried shrimp, fried chicken, veggie sticks, onion rings, things of this nature.

Burgers. Pizza. Handheld food.
Anything quick. Or anything you can have fun making together.
Like jello/gelatin. Brownies. No bake cookies? Maybe even your own icecream.
Definitely nothing messy. Long. Or complex of a cuisine. No octopus unless you’re both feeling daring and want to spark it up.
My ideal food is a Japanese sushi platter or a Korean appetizer sampler with beef and rice. I would only do that on a special occasion because it's so expensive.

Really depends on the person I’m taking out on a date, athough I have taken a vegan to a steak restaurant, on my defence she never said. It was a short date. Also it might work in Hollywood movies but taking someone on a date who is in a well paid job, does not always work out well hitting some local chain pizza place like domino’s. I tend to take a girl to some place we can both chat, not run the risk of either of us getting hit on by others while put, standing at the bar getting chatted up by another girl is not a good look on a date llol.
Some place where the server doesn't ask "how's it tastin'?"
For a first date I'd go for sushi, gourmet, steak. After than something more casual like an italian restaurant is fine.

There usually isn't any pre-planned meals. It depends on what the two people agree on. I've found that on the first date the woman usually eats like a bird and just orders a salad and soup
something which dont stick to your teeth, not messy, and make your breath stink (and the other end too)
Anything that’s not messy or gives you bad breath so you’re probably good with Pizza
