I haven´t been to church for about 20 years. I was taken to Sunday school as a child but I found it all rather silly. I stopped going at 11 and have never looked back. Now I live in Seattle and none of my friends are religious. I didn´t pick them for that reason, that´s just how it is in Seattle. It´s the least religious city in the US.
Would the most religious be either New York City (Irish Catholics) or that city who's name I've forgotten with lots of Mormons?
I don't know, it could be Salt Lake City, Utah. About half the population, maybe a bit less, are Mormon, but of course a fair number of the rest are Christians of some denomination. I expect the most religious cities are in the deep south but I could be wrong. The professional communities of the west coast and New England are generally secular to the point that it is unusual to meet someone who attends services.

Besides a wedding or a funeral I haven't been to church in over 30 years.
I go weekly unless I'm really too sick to be around other people -- in which case I follow along online. It's part of my weekly reboot.
I think it was a funeral. Long ago
Luckily no one has died since.
Every week, unless the rare exception of an emergency or vacation crops up.
