Why is it that the leading cause of death in pregnant women living in the US is homicide?

Because they're too young to die of anything else. The vast majority are going to be under 35, so they won't be dying of heart disease, cancer, strokes or any of the other big killers.
It actually reflects pretty well on the healthcare system, that they're not dying during childbirth or from other complications, as is too often the case in less developed parts of the world. Plus, women (especially pregnant ones) typically don't do dumb stuff like ride motorbikes or drive recklessly. In fact, most heavily pregnant women barely leave the house, so there's really nothing else left that they *could* die of (and they have to die of *something* to be included in the statistics).

Women in the United States are 2 time more likely to be killed in a homicide than die from pregnancy related complications like hemorrhage. The United States also has one of the highest maternity mortality rates to pregnancy related complications than most other high-income countries, in fact it almost double that of comparable countries.

Well, if it were just a gun issue, we would see a lot more shootings by women, but instead we see 90% of homicides committed by men, while only around 2% are committed by women.

I assume it is to do with the fact that women who are pregnant are younger so illnesses that cause death to older people are less likely to be a problem and also due to them having regular check ups during their pregnancy, diseases are less likely to get to a stage where they are fatal. Therefore unnatural causes such as car accidents and murder are more likely to be causes of death as they cannot be prevented by medicine or medical screening and they are not at a stage in their lives where other conditions would be fatal.

Because pregnant women are young, which means the other leading causes of death are unlikely. Being pregnant doesn't increase their chances of being killed by homicide, it decreases their chance of heart attacks, strokes, diabetes, upper respiratory infections, etc.

I did a little bit of digging into this. Not a lot, but enough to find out some things. There are a lot of negative and misleading ideas with this. I'm NOT saying it's not true. I am NOT saying it's not a problem. But there are definitely problems with the study. Some of the following is just informational.

1) The study was a very poor study using very limited and unreliable data. Bottom line is that good data on this doesn't even exist. It's not even tracked. Only recently have some jurisdictions started to track it, but even that does not use good methodology. Good data simply doesn't exist.

2) "Leading" cause of death does not mean the majority. It means the plurality. Only a relatively small minority of deaths of pregnant women are due to homicide.

3) Numbers from different studies vary by more than five to one. Numbers that vary that much are a sign of poor studies, and/or bad data.

4) Homicide does not mean murder. I don't know what definition the study used for homicide. Homicide includes suicide and some auto accidents, and possibly other types.

5) Suicide and drug overdose cases outnumber homicide cases, possibly by quite a bit.

6) This accounts for about 3-4% of murder cases of women.

7) Most murder cases of pregnant women are due to domestic violence.

8) The numbers include ALL pregnancies. I don't want to get into the abortion issue - but draw your own conclusions.

9) Some studies show that homicide is not even the plurality of deaths of pregnant women. They show that accidents are the leading cause of death for pregnant women and all women in that age group.

Again, I'm not trying to downplay this. I'm just putting it in perspective, and stressing that good numbers are not known, and there is currently no way to get good numbers.

I looked it up and the answer is two-fold. On one hand, the homicide thing is more of a comparison. It's like when it was said that young people were more likely to get struck by lightning, than die from Covid. It''s not about getting hit by lightning, or why you would get hit by lightning, it's just a comparison.
In this case, the comparison is between the pregnant woman dying of homicide, versus obstetric causes.
But according to the Harvard study, these homicides are occurring because of a "deadly mix of intimate partner violence." So these aren't random killings by strangers.

But after reading the study, I've concluded that this is all about restricting abortion. What other purpose would there be to this study?

An in-depth qualitative study examining abuse patterns during pregnancy suggested that for approximately one-third of battered women, pregnancy was a protective period, while for another group (approximately 15% of those abused during pregnancy), abuse started or worsened during pregnancy often due to partners doubting the baby was theirs. However, abuse patterns remained consistent for the largest proportion of women abused during pregnancy.17”

If the homicide rate is higher while the prevalence of domestic violence decreases during pregnancy in twice as many cases as it increases, I think the best explanation is that physical vulnerability during pregnancy resulting in a higher fatality rate when violence occurs is at least partially causative.
