Yeah a few times, but not in the U. S. that I can remember.
About 40,000 people get shot every year. There's nearly 400,000,000 privately held firearms in the country. That means at most only 1 in 10,000 will shoot someone, but that number is further reduced (drastically) because of all the gang violence where one person makes multiple shootings.
So no, it's not a common occurrence for gun owners.

No, not even close. In fact most cops and soldiers never will either. The overwhelming majority of firearms in the US end up never being fired anywhere other than a shooting range, or collective dust in a safe or locker of some kind.
You're not truly an American until you've shot someone (or at least told a homeless beggar to, "Get a job!").
