Mental illness doesn’t mean everyone completely loses their grasp on reality or their moral compass. Autistic people, for example, have been known to do good things more often than those not on the spectrum because they so strictly adhere to their moral compasses that they were taught. Yes, a person in the throes of something like a schizophrenic episode might have a hard time knowing what’s right and wrong when they’re hearing and seeing all kinds of things you and I can’t fathom but that doesn’t mean they don’t know what good and bad is, they just lose the world for a little while. Now, someone like a narcissist or a psychopath might know right from wrong and not care as it’s not something that ultimately benefits them but that’s a separate issue. The point id being diagnosed with something is not a sign that you’re an inherently bad person, nor is it a sign that you lose your grip on reality. It’s the same as saying a herpes diagnosis means you’re a slut — herpes is a skin condition that’s an offshoot of chicken pox, it has very little to do with sex beyond being spread by fluids like saliva. Talking to someone with herpes for a long enough period of time in close quarters that microscopic bits of saliva could fall into your mouth and be ingested by you is enough to also give you herpes. Didn’t think that needed to be spelled out for you.

Now, I know you may think you're just asking a harmless question, but let me tell you, my dear, that words have power. And in this case, your words have the power to harm and further marginalize individuals with mental illness. So, I suggest you refrain from asking such questions in the future and instead strive to be a positive force in our community.
