It's called reciprocity. Most states share information for treatment purposes. If states block Texas from receiving treatment information, then Texas blocks other states from treatment information. As a result many patients will not have accurate health records when moving or traveling. This can lead to worse healthcare and potentially increase malpractice law suits. Texas being the second most populous state has a bigger stick in the fight than most states being that if they don't share health information, other states will do a worse job.

Technically both, but it goes well beyond just doctors. There are healthcare providers which include doctors, but also include other staff like nurses and entities like hospitals, clinics, pharmacies, and even nursing homes and mental health facilities. Additionally you have government health programs like medicare or medicaid which often are state funded up to 50%, public health clinics, state departments of health which track vital records, immunizations, etc. There are also databases for mental health evaluations. Even health insurance companies are able to receive and transmit some data. They have to follow HIPPA rules, but information pertinent to treatment can be transmitted without a patient signing privacy releases.

If you don't want the state to be able to see your medical records you basically have to vote against all social safety nets dealing with health care, you have to repeal centralized health insurance portals such as Obama Care, you have to remove mental health background checks for firearms, etc. So pretty you have to vote against the Democrats. The more you go toward socialism, the more the state has access to your information. Right now it's decentralized with the state having some information and private providers having some information and exchanges happening for patient care. If you go down the line of nationalized health care then you have centralized databases in the hands of the states to see everything all the time.

There is NICS mental health reporting for background checks. Generally law enforcement has to subpoena medical records if it is part of an investigation. If you have been convicted then the prison system can get medical records because you are now a patient of a state hospital. Therefore they may need to know your medical history, medications, and mental health record in order to treat you.

As far as abortions specifically though, that would concern the department of health. Births, deaths, miscarriages/abortions, marriage records, etc. fall under vital records. So if one state refuses to share if an abortion of a child occurred, then the other state could just as easily refuse to confirm or deny birth records or marriage records leaving a person in another state unconfirmed as an actual person with legal standing in the United States. They could just say we have no records of that person existing. Good day. So states have to kind of play ball with each other on that matter.
