Obtaining nuclear weapons is expensive but maintaining them and keeping them safe is even more expensive. Most corrupt countries which are always on threshold of internal destabilization are incapable to create proper nuclear programs. In Venezuela every commander who would got the order to guard nukes would start a coup in no time, I guess this is a problem in Iran either. Another problem is that modern nukes need Tritium that has a half time of 12 years. Ergo you need to replace it all few years and you need to get this tritium gas from somewhere. You need an breeding reactor that costs a lot to maintain such a nuke. Without enough tritium such a nuke won't have enough slow neutrons for chain reaction and just turns into radioactive and poisonous dust when conventional explosives compress the core.
Without tritium there is an option to build heavy bombs like in 1950ties but to drop a several tons heavy bomb is a fleet of strategic bombers like Tu-95 or B-52 necessary, which will be very likely shot down before they can reach their target.
