Are you in vanish mode?
No idea
If you're in messages in insta, tap on the person's username. It'll open up details. The second option down should be Vanish Mode. My understanding is that if that's on, your messages get sent and seen but vanish when you close or something like that. This may be what you're seeing. It's really easy to turn on without thinking about it from inside messages itself just by swiping up so you can do it by accident.
