It means that the person who apologized after months is using the other person as a backup plan because they can't find anybody else. Someone who really wants to keep a relationship with someone will usually apologize pretty soon--they don't wait months. If he waited months, it means that he probably thought, "It's fine, I'll find someone else," went off to check any available partners, didn't find any, and then proceeded to go back to his ex because he has no one. The fact that he didn't apologize meant he probably thought you were replaceable. However, I may be assuming too much, and you're the expert in your own situation, so just take my opinion with a grain of salt because there may be some other details I do not know of.

He definitely wants something out of you. But if he waited months and could live months without you and now thinks about you, it's probably because nobody was interested in him and he wants someone who he knows will get back with him. That's how on-and-off relationships work: they're really unhealthy.

It's your choice on what to do with the relationship. If he did something bad and didn't apologize for months, it shows how much he really cared about the relationship... But then again, details of the relationship are kind of vague, so I can't be entirely sure about my answer.
