What he has threatened is the supreme manipulation and, if you allow this to be successful, you will be surrendering control of the rest of your life. NOT a good idea!

Tell his family what he has threatened, and ask them to standby while you deliver the break up message, and then they can step in and take responsibility for his safety.

Call his bluff. Do it. Any boyfriend who says that: a) shouldn't be your boyfriend (you deserve better) and b) is going to hurt you (probably physically) in the future. End it and get far away from him.
