They become distant, don't want to spend time together, basically they stop trying or putting effort into the relationship. This makes the person feel unloved and thus eventually fall out if love.

Time and experience. We all change over time, and everything around us changes as well. Sometimes love between two people survives these changes, but most of the time it doesn't. And it doesn't necessarily mean the love was weak. It's just how life goes.
