Not necessarily competition wise, just hard in terms of responsibility of the role
I'd say being a cop or front line combat in the military. Those are the only 2 jobs that ruined any of my friends' minds. One of my friends was a cop out in eastern Wyoming. He was the first responder to over 100 suicides and murders, mostly suicides. That shit really fucked his mind up. Add being the human garbage man and dealing with people that hate you everyday, going to people's houses to arrest them in front of their crying kids, taking their kids away for abuse or neglect, wondering when you're going to get shot off somebody's front porch, etc. That shit adds up.
Another friend went to Iraq and killed a bunch of Iraqis. He's a good guy and didn't kill anybody who wasn't trying to kill him, but that war ruined his mind. Paramedics and firefighters also see a lot of death as first responders and are repeatedly traumatized by it, too. So are emergency room nurses, doctors, and other workers.

Physician. The level of knowledge needed and the level of responsibility. Red tape, required paperwork. There can be very long hours and being tired. But I"m sure others on the above list can be equally difficult at times.
