Well, the Disney race swaps are the low hanging fruit, but most of those remakes shouldn't have been made in the first place regardless of who plays who.
Ezra Miller as the flash stands out. He was a terrible choice for that role even before he went crazy. This one really stands out in my opinion because almost every other casting decision in the DC live action movies was good, but those movies were plagued by other problems. Marvel generally had decent casting too with a few scattered duds, but have been consistently mediocre or bad with characters introduced post-infinity war.
I had forgotten all about Tom Cruise as Jack Reacher - that one was really bad too, even as a Tom Cruise fan. The same can be said for the Mummy reboot.
Probably my most controversial entry though would be Daniel Craig as James Bond. He was an awful James Bond with exactly one great movie (Casino Royale) and one decent one (Skyfall) and the other three are completely forgettable despite having generally good villains and supporting characters, and Craig himself being a good actor.

That weenie kid, Asa Butterfield who played Ender Wiggin in "Ender's Game".
In his defense, the director of the movie made Ender seem like a nice guy - a weenie who just has a killer instinct in tactics, so perhaps Butterfied - a weenie - was a good choice to play a weenie. But Ender really isn't like that in the books.

Michelle Geller in Cruel Intentions, it was one of the worst acting jobs I've ever seen, as well as Salma Blair in the same movie, also a very bad actress for her role. I think if they had cast Jennifer Lopez instead of Michelle Geller and replaced Salma Blair with Lucy Liu, they would have had a good movie.
Almost everyone in American Satan. Especially Andy Biersack as the lead... like jeesh, this could've been fun and dark if you had capable actors (and writers), but it went for mediocrity. And sometimes below average. Like c'mon... why casting Drake Bell? Really?

Bella Swan from Twilight. I always thought Emily Browning should have played her not Kristen Stewart. Though she did grow on me as the series went on but she's still not my first choice.

Definitely Jafar in the real live version of the movie Aladdin and yes, I kind of want my cartoon version characters to match the real live version characters. It seems like he had tiny man syndrome.
Oh definitely Hoffman in the SAW movies. The character was okay I guess the actor that played it was not.
Still a good movie, but Mickey Rooney (whom I love in many other films) in Breakfast at Tiffany's was a disaster. Benson Fong would have been an INFINITELY better choice as Mr. Yunioshi.
