No, you dont wear anything under a wetsuit. Creates air bubbles which can be dangerous under the wrong circumstances

I wear a bikini but its more so i get take the wetsuit off in public without being naked.
I usually wear a bikini or regular underwear under it, sometimes even a tank top.

Also depends if you need any support up top, but yes defiantly wear something under even a one peice so there is no straps to come undone.
