there are two types of surfers, ones that wear something under their wetsuit and those that dont. i guess it depends on a lot of things like, where do you get changed if at home then no problem if you have to put the wetsuit on in a public place like on the beach then i would suggest a swimsuit under neith. also if you are planning on playing volly ball afterwards i would guess that you may end up rolling down the top half and at which point you would need something on under it.
i would suggest a one piece swimsuit as they tend to have less underwires than a bikini and also you won't have tie knots etc for doing up the top half. and you want what ever is under it to be as close to a second skin as poss as that is what the wetsuit is and any lumps and bumps under it will annoy you by the end of the day.

The general consensus among most divers is not to wear anything under a wet suit as it is likely to bunch up and feel uncomfortable and cause chafing against a larger area than just the suit alone.
However if you do not have a place where you can change then you are pretty much stuck wearing something. In that case a thin, stretchable material like spandex is best rather than looser fitting fabrics like cotton.
On land you could get really hot so having the option to roll down the top would be useful.

It really is up to you. I think it would be better to wear something underneath so that you can change in and out of your wetsuit in public if the need arises. (Otherwise you would have to do some really creative gymnastics with a large towel - not that it isn't impossible.)
Also, if you're going to be wearing it all day on a hot day, you might want to take off the top half for comfort and then you'd want something on underneath.Yes, especially if you're planning to play volleyball.

Really depends on if you want to be able to take it off and not be naked underneath. If that's not an issue, then no need for a bathing suit underneath. You could also just wear bottoms and when you take it off you can put the top on if you need to.
You don't have to but if you are going to be doing anything where you are likely to take it off I would suggest it.
