The by-the-book thought would say low.
However it's half and half I believe. Around half of these women don't suffer from low self-esteem. On the contrary, their body and the (let's use the frank definition here) ability they have to show it in dick-raising poses, and the resulting compliments, get them addicted.
It can even reach the "goddess complex" stage, which I don't think is the case here at all, but we are speaking of the broad picture.
