The tat is their personal thing. But it does open up dialog. It's the wrong way to do it for one and I might point it out for fun.
I just met a random 70 year old woman in a parking lot the other night. Ended up talking to her for 45 min, she was full of fascinating stories. One her husband saw someone ride their bike into the flatbed of a tow truck and was decapitated.

Usually tatoos means something for those who get it, so I would naturally think it is crazy but then my conscious mind would be thinking, what is the reason and why did they get it in the first place and hear them out. No matter what, they are the one living with it. Not me!

That's too low anyway, so I'd think they were a dumpster fire of missed therapy appointments and bad life choices but can't even be logistically accurate. That's about two inches lower than anyone would ever cut. Why would you risk having to scrape off the collar bones FFS?

If it was a pretty girl, then I would grab a non-permanent marker and add an "ey" to the end of cut. I would say, not everyone feels the way you do about yourself. Maybe that would make her smile a little, at least in the interim.
