Context matters.

If someone randomly asks a person if they have had cosmetic surgery, in most contexts, that is rude in itself and a rude reply is to be expected.

If two people who know each other are having a discussion that touches on health or beauty, then the question is related to the topic of discussion and not rude, and doesn't deserve a rude response.

Regardless, most cosmetic surgery is not difficult to detect if you know what to look for. I can almost always tell, and I don't need to ask anyone.
