Great question, I always took it to mean on the surface the person had issues. But since you asked I suppose it could be a compliment meaning the person wasn't just nice on the surface, but was genuine and not one to smile at you while talking behind your back.

Well, I don't care for it at all because it's just not true. There are no issues with her, he's just making things up!
It's not a very nice thing to say about a person- especially when the person saying it to other people cannot give her even ONE example of what she did that is actually true.
