It's not really the boob what hurts but what's inside: the mammary gland.

Mammary gland is very sensitive to pressure. Depending on the size of the gland and how much fat there's around the boob, how soft or hard is that fat, size and so... it hurts more or less and it's easier to take a punch there or not.

I haven't seen that much people hitting women breasts, but I know that to practice a mammography is necessary to squeeze the boobs putting a lot of pressure... and there are women who go through a really bad time when having one, and women who can take them pretty well.
