It depends on the context. I very rarely refer to a women as "b****". But I do frequently on this site or referring to site. But to be fair, this site has a lot of b****es. If I'm using that term the "woman" has generously earned it. I'd prefer to never say it.

I just look at it like slang. In America saying the word "cunt" is forbidden but in the UK they say it all the time
