I don’t have much money. My mum’s friend had a genetic disorder that made her really fat, like an Easter egg. I haven’t seen her in years and she recently had surgery, which could’ve killed her, but she survived. Removing all the Easter egg shaped fat. Should I buy her something? Something small, like a bottle of wine, or a piece of jewellery? Or leave it?

But wine and jewelry even small stuff, still costs some dolla bills. And you said money's tight which is valid.
Maybe you could make her somethin sick crafts-style from stuff round the house instead? Like a get well card with some dope designs you cut out or sumthin. Chicks usually dig homemade gifts more anyway.
Or ask your mum what her friend's into - maybe you can score a cheap used book or playlist on her favorite music for like nothin on eBay.
Just the thought you cared enough to give sumthin means a lot. And saves you coin too so win-win ya feel me?

Never give gifts if it's a financial burden on you. Most people would not want a gift under those circumstances anyway.
If it's not a financial burden, it depends on whether you want to give a gift. In your case it sounds like a financial burden to buy a gift. Maybe you can find something that's free, or make something yourself, or just send a card. Or simply spend time with her and show friendship.
If you want to, sure. I doubt she’s expecting you to get her anything so don’t feel like you absolutely have to.
