Oh... yeah, I think you did everything fine. She has her work schedule, so it's not like there are lots of different times you'd be meeting up. Applebees is usually a dinner spot, so it makes sense that she would be meeting you for drinks after work.
If you wanted to meet for lunch you probably would have specified that.

That is a trickier question... on one hand, it's probably not worth getting hung up about, but on the other, it shows a certain lack of respect for your time.
I would probably wait until after you see her again in person to text. And I don't mean to just automatically text her some mindless thing after you see her. Probably see her in person. If she brings up the Applebees thing, probably just laugh it off and try make an established plan right then and there.
Most people are space cadets and lost in their own little worlds. It's good to show those people grace. Some people are just disrespectful and couldn't care less about making plans with other people; those are the people you want to get rid of.

I would say it on your friend. I guess she is one of these people that you have to make plans with days in advance.
I would say she is wrong.
Some people are inconsiderate like that. She was probably getting fucked by her boyfriend and/or getting high.
It most have been too late for her. Either way, she should have given you a proper response to let you know if she could make it or not.
Some people will just agree to agree. However, she probably never wanted to go that late or maybe she never wanted to go at all.
Yes, but people will do that because they know that is what you want to hear. They think it makes you happy to hear yes, but they know their intentions of going were never there to begin with.
No, I just think she thought you would be disappointed if she said no. Therefore, in here mind she thought telling you yes would make you happy.
