I hooked up with this guy for the first time and it was a really fun time but he only lasted 15 seconds each of the two times we actually had sex. We made plans for later but he flaked. It sounds like you're extremely attractive (at the very least he seems to think so).
So he's nervous about performing in bed, and he's extra nervous about having performed poorly, and he's extra extra nervous about what you're gonna think of him after all this.

You said it was a really fun time. Tell him that. Directly. Do it in a way that tells him you want him to know that you had fun and that you liked it. And that you wanna do it again. Let him know that you want him to try again, regardless of how long he lasted, and regardless of how embarassed he is. Be patient. But try to tell him directly if subtle doesn't work. Let him know he doesn't have to be scared. But also let him know you'd like him to take initiative again, or respond and not hide (if any of these are the case :D).

Next time you have sex, try to not have the sex end just because he ejaculated. You can make out. You can play with each other. You both have hands and lips. :D
Maybe tell him beforehand that, because you like having sex with him, and you don't care that much if he comes after 15 seconds, that he shouldn't stop just because he come. He shouldn't stop because he's embarassed about it. Both are OK.
But if any sexual encounter ends with him being embarassed, it's only gonna get worse everytime you try.

So, first try getting him comfortable with trying again. And, second, when you try, try to stay in it, and end it on some kind of successful note.
