What does this mean. It’s like a hard pregnant stomach and you can’t grab any fat. He used to have a six pack about 8 years ago when I knew him. He isn’t really fat anywhere else except his stomach. what could cause this and what could it mean? I’m actually concerned.
Sounds like his liver is in pretty bad shape.
Likely due to excessive alcohol consumption at one point.

It might be just water, make him go to a doctor, if he got a build up of fluids in his body. They can simply drain him.

We're guys. We are genetically programmed to store excess weight on our bellies. In most instances, this is one of the main physiological differences between men and women and that may be why SOME women actually like it.
If his belly is hard and big and it's always the same size rather than looking like he's just bloated (which would cause his belly to vary in size, day to day), you're probably dealing with your average guy who has a gut composed of visceral fat which builds up under our ab muscles and abdominal wall and pushes those out forming your classic "guy gut". If your boyfriend's belly were soft, it would indicate subcutaneous fat, which is only under the skin. Over the last decade or so, doctors have quoted research that says that visceral fat builds up deep in our bellies and if there is enough, it can start affecting our health because it is biologically active and right around our stomachs, livers, etc. Subcutaneous fat is much less of a health threat to us. On the other hand, doctors say a lot of things and if you notice, most male doctors who say this often have a gut themselves! Everything comes under scrutiny today and I know guys who have had a bit of a gut their whole lives and are fine. Yet, it's probably wise to err on the side of caution and get him to exercise, cut the sweets, too many carbs and the beer and he will probably be able to shrink his belly to a size that strikes the balance between health and how he is comfortable. Having a six-pack on your belly is not really an easily attainable goal for anyone who is not obsessed with his body, so you probably should not expect that. Shoot for a little bit of a Dadbod and encourage him to have regular check-ups and love him for his physical characteristics and try to accept and enjoy one of the physical difference between men and women. ;-D
