When should I see my GYNO for heavy bleeding and severe cramps?

I normally do not have cramps due to being on birth control.. its very rare that i have them and when i do, its mild. Today it hit me out of nowhere.. from days 1-4, I was just spotting wit brown discharge, now I am on day 5 with a super heavy flow and severe cramps to where its almost unbearable while I am working. I took Ibuprofen and its not helping a whole lot. I am light headed, and fatigue... also if I need to make an emergency appt ASAP today, how do i tell my boss that is male?

Since I started working in this office, I have had having numerous dr appts, procedures etc that was requested off in advance but never really had to leave in the middle of the day at work.
