I deal with all three and what I find helps is having a good support system to get you through it, gabapentin and hydroxizine for the medication side, and (at least for me) trying to repeat a mantra in my head that isn't "your trash, nobody likes you" (I have really bad self esteem)
Talk to a dr and get some help i take a few different things to help with all of those thing and have been self harm free since 2018
Visualization and breathing can help with panic attacks and anxiety. Imagine inhaling and clean clear water washes over a rag in your hands, exhale and you are wringing out all the dirty water.
You need company and breathing exercises
You should also see a doctor they can help with your case
You also need to fight yourself and force yourself to go out

yeah, get off social media completely. Exercise. Eat healthy. Get sunlight and time in nature every day. No video games. No porn. Read books. Get a job. Work hard.
When you're having a panic or anxiety attack, eat some ghost pepper, Carolina reaper, or Trinidad scorpion pepper
Just get over it! My motivations were verbal and even sometimes physical. Kicked my teenage ass in gear like 30 years ago.
Get healthy and gain confidence/self worth. Go join a jiu-jitsu gym
See someone qualified to treat it.
