I doubt that she was thinking about *things* when she saved you, so big things probably aren't the answer. What she'll appreciate the most is a very warm, heartfelt thank you in a card or a letter. Make sure she understands how much it means to you that she did whatever she did for you. Did she put her own safety at risk doing it? If so then she's a heroine as well. Since it's also a birthday, a small token isn't a bad idea. Someone else suggested jewelry and that sounds nice. A tasteful charm bracelet might work, especially if it has a charm on it that somehow reminds her of what she did. Don't spend a lot since the real gift will be your feelings in the note. Whatever you do, handwrite it! I once got a supposedly heartfelt note from someone and they had typed it, printed it out and folded the paper into the card. Seriously lame.
There really is no gift with that kind of significance, I know you want it to be special, I am sure. I personally would write a letter, telling her all the positives life has brought to me in the last two years, I am sure you would make her feel more important and special than getting her cookies or a gift card!!! What DO you know about her in terms of interests and such? Anything to work from? If not, you're kind of stuck being a bit generic with flowers or something, but I'm sure she will appreciate the gesture regardless!

What about a gift card? That way if it's money, you give her the freedom to do whatever she wants with it.
Along with a card expressing your gratitude so she can know the extent of how appreciative you are of her help.
Flowers or chocolates, but honestly I think anything you get she will appreciate. That's really nice of you to do by the way.
Food. You can't go wrong with food. When I help someone they usually give me food (chocolate, candy, etc...) or they invite me to apero / diner.
