What is the biggest mistake you have ever witnessed?
I've been so fortunate in my career. All the accidents happened to the other guys. And not near me. It was always when I wasn't nearby. Two parachute deaths. One guy lost his hand and another almost lost it when explosives went off unexpectedly. Lots of fatal car accidents.
In Afghanistan we were about to roll out the gate to the helicopter pad and I saw something shiny in that fine dusty dirt. The sniper that was supposed to guard the commander on the assault had been doing a last second cleaning of his sniper rifle... and dropped his firing pin in the dirt. I found it. Of course that turd blamed someone else.
Again in Afghanistan I was the base ops officer at our firebase. A platoon of infantry had just arrived and they wanted to confirm their sights on their guns because they'd come up to our elevation and they needed to know exactly where their sights were set. We had an area designated for that in an empty little valley at the entrance to the mountains. I heard a massive barrage from inside the command bunker. I wasn't expecting that. I ran outside and climbed on top of the bunker and saw that they were shooting in the wrong direction. I called for an immediate cease -fire. There was a small village behind a small hill, but that hill was not where they were supposed to be shooting. So they ceased fire. I told the commander and told him we needed to make sure no one got hurt. So we sent out a patrol heavy with medics and interpreters. No villagers were hurt at all. Fuqin lieutenants...🙄
Can we just skip the butter bar stage and send them out when they have at least a bit of common sense and understand they aren’t god? I had one good ensign on the boat, he got it and actually let us do our jobs and we made him look better in the process.
@Sixgunsound I think we should eliminate the officer corp as is completely. EVERYONE comes in as a private. They all pull KP and guard duty and dig slit trenches and low crawl through mud and just deal with the shyt life of a private. Once you make E-5 you test out. If you already have a commission then you go to OCS. If not then you go to college and get a degree associated with your branch and then go to OCS and then your Officer Basic Course. I detest officers. It's a throwback to nobility buying a commission. You can send others to their deaths simply because you have money. That type of authority should be earned and paid for with their own sweat and blood. There are a few good officers but the majority are political hacks concerned only with their own ambitions and not service to the nation and the preservation of our own daughters and sons in uniform.
years ago when I was working at a food service company. one of the supply guys was asked to bring to the service personnel a bag of salt. The moron delivered salt for the dish washing machine. as if this wasn't enough, the other morons actually served that chemical salt with the food. All guests ended up with red, swollen throats and terrible pain.
I saw a truck crash into another vehicle near us while riding the bus, I think the truck broke into two pieces but the rider got out without dangerous injuries or visible blood and I consider that an act of God cause a woman on the bus was chatting with us about how she worked for a Christian charity when the car crash event happened and nobody was injured. Like a guy died in a motor cycle crash in front of our church when I was 14 and the pastor asked us if we wanted to ask him questions and a guy in my group said he wanted to know why Gods let him die in front of a church.
Watching a childhood friend accidentally kill himself. I was walking through town late at night and I saw him dropping his friends off at the pub before proceeding to do skids/burnouts and show off in his truck on the road. He didn’t turn hard enough and drove straight off the town bridge into the river. A few people jumped in the river to get him out but his door wouldn’t budge so there was nothing that could be done. This was on Christmas Eve a couple years ago. Emergency department showed up 20 minutes later and fished his car out, but it was too late to save him.
Besides electing Biden as president of the free world I was at a funeral and the mortician parked the hearse on a hill and did not secure the parking brake. The hearse rolled downhill, hit a tree, the back door opened up and the casket ended up on the ground. Fortunately it did not open.
At work, someone messed up some custom work for a client and it ended up paying them out over a million in compensation so they could hire people on their end to manually fix the problem.
That isn't even the most expensive mistake that has happened, just the most I've been part of as it escalated to me.
It is cause they are too cheap to hire enough help and get decent hardware, they'd rather pay out millions in compensation than a few hundred thousand in enough help and hardware.
During a fireworks show at the beach, one of the guys on the barge knocked a lit firework over.
It exploded sideways which set the boat on fire.
All of the men jumped off the barge and the most intense firework show on earth happened.
(Emergency boats were already standing by and everyone was fine)

Being born.
But I am correcting it ever since and am not giving up. Making my own money. Claiming my autonomy. Instead of getting my ass whopped by life I am turning it to the opposite. Soon I will be whopping the crap out of life for daring being shitty towards me. Life will soon deeply regret doing all those things to me.
I witnessed a foreigner being quite rude to a waitress... while a cartel persona was a couple of tables next to them... that's a least a couple years of physical therapy x 4 limbs
Coworkers leaving in a storm they had no business being in. Rookie deckhands didn’t know any better and I don’t know what the captain was thinking. Wasn’t nice getting the call that they’d gone down a few hours later
Biggest mistake I saw was a flight deck crew forgot to remove a large rubber strip from a aircraft carriers catapult before commencing night operations. An F-18 was launched and the strip was ingested by both engines, killing one immediately and badly damaging the remaining engine. The aviator hung tough and got the aircraft back aboard via the barricade. Probably $10M of damage when all was said and done.
I am wondering about that too. I personally put diesel into a gasoline engine (with turbo). All they had to do was to drain the tank, clean fuel line and fuel filter. Cost me 75€.
