This wasn't exactly a mispronounced name but a funny story about someone's name.
When I was a secretary my boss was out in the field when a customer called on the phone for him.
I asked him if I could leave him a message, he replied "Yes, tell him that Hy Weiner called".
I laughed and said "Ok, now tell me your real name" and he said "That is my real name"!!! In a very stern voice.
Oooooops!! 🙄
I'd always double check how to pronounce first because it's only common decency to do so. That said Siôn and Sean are really common names around here but you'll hear outsiders pronouncing them wrong all the time.
only annoying ones... from government institutions that keep butchering my last names

my both names didn't come from the language of the country I was born and grew up in... so... it was funny at school and public offices :D
also... as a kid, in primary and middle school, I didn't learn English (it was an evil language of capitalistic countries), so being in high school, where English finally was introduced after changes in 1990, I needed to read a text about Sean Connery... you can only imagine how badly I messed up with his name :D
Same point as my question. Sean is one of the world's most mispronounced names. Even in the UK you'll hear people rhyming it with bean. To confuse matters Sion is pronounced the same as Sean and Shaun
