Why did he call me and text me this after when we have been over?

I dated a guy for almost a year. End of April our relationship went downhill, he started being super mean towards me and would bring a girl over and it hurt me. he's a narcissist and says he did nothing wrong and if anything did so many good things for me, but I have mostly bad memories of him being angry at me for nothing. Last night he called me and said everything is going wrong for him and that he fell the other day and tore his muscle and now needs surgery. I didn't like how he kept saying he wants to end his life. I kept telling him dont do it but he said he really thinks he is. The part im afraid is I have to go return his house keys but im worried he's gonna decide to do something that day and I dont want to get traumatized. Before hanging up phone I told him take care. An hour later he sends me 2 photos of reasons why a relationship won't work. I can relate to number 10 and he relates to 9 and 11 but why is he saying this? I dont care anymore about why it didn't work I just dont want him doing something stupid

He's starting to worry me he texted me again at 1am saying he is so done with his life. I told him call me so he did and he was telling me "whats the point of me living when I have so much pain with the 2 torn muscles in my body I can't even function right and can't be sane can't do my favorite hobbies because I can't walk or ride my motorcycle from being in so much pain whats the point!" He started punching his face and he was crying. I told him please don't think like that. I started crying
