WTF does it makes me so pissed off - when I see my girlfriend's ex liking all her pics and even she liking all his pics?

I would probably feel the same as you. I would honestly tell her it's bothering me and that I'm not comfortable with her following he rex and exchanging likes with him.

No it’s not ok. And I know from experience it’s a huge red flag. I’d tell her to stop and if she didn’t or made any sort of big deal about I’d dump her.

Not ok. If she doesn't respect your boundaries once you reasonably set them, you have problems to solve and decisions to make.
