It can actually be either of those. "Transgender" means essentially that the brain and the body are in disagreement on what sex they're supposed to be.

Let me see if I can explain... When a child is born, a doctor says, "It's a boy" or "It's a girl." right? That's called "assigning the sex," which is based on biology -- chromosomes, anatomy, and hormones. And USUALLY this is correct.

However, because the body and the brain develop at different stages in the womb, sometimes a person's gender identity -- the inner sense of being male, female, or both -- doesn't match the biology. Because male and female brains are slightly different, you could, to put it VERY simply, say that you have a female brain in a male body... or a male brain in a female body.

So these people grow up knowing that their body is WRONG. Their brains tell them that they SHOULD be something else than their body tells them.

Some of these people get sex-change operations so they can live as the gender their brains tell them they are. Some of them don't. But all of them count as transgender, because they were born the "wrong" gender.

Generally not, no. A lot of people try to blame it on the parenting, but far as I know that doesn't have much to do with gender identity, if anything.

That's called being "genderfluid." It means your mental gender is somewhere between male and female. Sometimes you feel more like the one than the other. Sadly you can't switch physical forms, but sometimes your brain is just fine being in your body and sometimes it just goes "nope!"
