If I witnessed this, not only would I be complaining to the restaurant manager about the unprofessionalism and transphobia of their staff, I'd be ringing up district managers and blasting the restaurant on social media.
Plus also aggressively be misgendering and snapping my fingers at the lowly garçon after that until I leave without giving a tip. (If my server is different from the transphobic waiter, I'll directly give them cash so they can slip it away without forcing a tip share).

Probably not. Part of being a waiter is being respectful and providing good service. Being a sensitive snowflake isn’t a valid reason to deliberately disrespect someone.
She shouldn't expect a tip from the person confused about their gender and the enablers they are eating with, but she should absolutely get a tip.
